Job Summary:
The Health Care Coordinator at Bluezone Care Ltd plays a crucial role in ensuring seamless coordination of care for clients. This position involves developing care plans, coordinating services, and acting as a liaison between clients, families, and healthcare providers. The Health Care Coordinator ensures that clients receive comprehensive and consistent care.

Key Responsibilities:

  • Develop and update individualized care plans for clients.
  • Coordinate care services, including scheduling and communication with healthcare providers.
  • Monitor and evaluate the effectiveness of care plans and make necessary adjustments.
  • Provide support and information to clients and families regarding care options and services.
  • Facilitate communication and collaboration among the care team, clients, and families.
  • Ensure compliance with all relevant healthcare regulations and standards.
  • Document care activities and maintain accurate records.
  • Assist with client assessments and the development of care goals.
  • Provide training and support to staff on care coordination processes.
  • Participate in quality improvement initiatives and audits.

Qualifications:

  • Bachelor’s degree in nursing, social work, healthcare administration, or a related field.
  • Experience in care coordination or a similar role within a healthcare setting.
  • Strong organizational and problem-solving skills.
  • Excellent communication and interpersonal abilities.
  • Knowledge of care planning and healthcare regulations.
  • Ability to work collaboratively with a multidisciplinary team.
  • Compassionate and client-centered approach.

Working Conditions:

Required to work 37hours and above per week Monday to Sunday, some days including weekends and holidays.

Care will be provided mainly in clients homes and other healthcare settings

Mandatory requirements

Qualifications

  • Must have completed a care certificate at the minimum or higher
  • Basic IT experience

Experience

  • Previous care experience, experience with working within a team in a healthcare setting
  • Able to demonstrate care and compassion
  • Be able to interpret and understand care plan documents

Communication

  • High proficiency in English language speaking and English writing (ILETS score of 6 and above)
  • Able to clearly document written information

Mobility requirements

  • Must hold a valid UK full driving license and have a car to use in this role
