Job Summary:
The Health Care Coordinator at Bluezone Care Ltd plays a crucial role in ensuring seamless coordination of care for clients. This position involves developing care plans, coordinating services, and acting as a liaison between clients, families, and healthcare providers. The Health Care Coordinator ensures that clients receive comprehensive and consistent care.
Key Responsibilities:
- Develop and update individualized care plans for clients.
- Coordinate care services, including scheduling and communication with healthcare providers.
- Monitor and evaluate the effectiveness of care plans and make necessary adjustments.
- Provide support and information to clients and families regarding care options and services.
- Facilitate communication and collaboration among the care team, clients, and families.
- Ensure compliance with all relevant healthcare regulations and standards.
- Document care activities and maintain accurate records.
- Assist with client assessments and the development of care goals.
- Provide training and support to staff on care coordination processes.
- Participate in quality improvement initiatives and audits.
Qualifications:
- Bachelor’s degree in nursing, social work, healthcare administration, or a related field.
- Experience in care coordination or a similar role within a healthcare setting.
- Strong organizational and problem-solving skills.
- Excellent communication and interpersonal abilities.
- Knowledge of care planning and healthcare regulations.
- Ability to work collaboratively with a multidisciplinary team.
- Compassionate and client-centered approach.
Working Conditions:
Required to work 37hours and above per week Monday to Sunday, some days including weekends and holidays.
Care will be provided mainly in clients homes and other healthcare settings

Qualifications
- Must have completed a care certificate at the minimum or higher
- Basic IT experience
Experience
- Previous care experience, experience with working within a team in a healthcare setting
- Able to demonstrate care and compassion
- Be able to interpret and understand care plan documents
Communication
- High proficiency in English language speaking and English writing (ILETS score of 6 and above)
- Able to clearly document written information
Mobility requirements
- Must hold a valid UK full driving license and have a car to use in this role
